Abstract
Healthcare systems worldwide encompass both traditional and contemporary medical practices. Ayurveda, an ancient Indian system of medicine, emphasizes individualized treatment, disease prevention, and restoration of physiological balance through herbal medicines, dietary regulation, lifestyle modification, and detoxification procedures. In contrast, contemporary allopathic medicine is founded on evidence-based scientific principles and utilizes pharmacological agents, advanced diagnostics, and technological interventions for disease management. Despite their widespread utilization, comprehensive system-wise comparisons between these medical paradigms remain limited. To critically compare ayurveda and allopathic medicine across major human organ systems, focusing on their philosophical foundations, diagnostic approaches, therapeutic strategies, clinical effectiveness, safety profiles, and potential role in integrative healthcare. A narrative review guided by PRISMA principles was conducted using electronic databases including PubMed, Scopus, Web of Science, Google Scholar, Embase, and Cochrane Library. Relevant studies published between January 2020 and March 2026 were identified using predefined keywords related to ayurveda, allopathy, integrative medicine, herbal therapeutics, pharmacotherapy, and system-specific diseases. Articles meeting predefined inclusion criteria were critically evaluated for clinical relevance, methodological quality, therapeutic outcomes, and safety considerations. The review demonstrates that ayurveda and allopathy possess distinct yet complementary strengths. Allopathic medicine provides rapid symptom control, precise diagnosis, emergency interventions, and scientifically validated treatment protocols, particularly for acute illnesses, infectious diseases, trauma, and critical care. Ayurveda offers a holistic framework emphasizing disease prevention, restoration of physiological balance, enhancement of quality of life, and long-term management of chronic disorders. Comparative evaluation across eleven organ systems revealed that ayurvedic interventions may provide supportive benefits in chronic inflammatory, metabolic, musculoskeletal, neurological, and lifestyle-related conditions, whereas allopathic medicine remains indispensable for acute disease management and advanced therapeutic interventions. Challenges associated with standardization, quality assurance, herb-drug interactions, pharmacovigilance, and limited large-scale clinical evidence continue to influence broader integration. Neither Ayurveda nor allopathic medicine should be viewed as mutually exclusive therapeutic systems. Instead, evidence supports a complementary and integrative healthcare model that combines the strengths of both approaches. Future multidisciplinary research, rigorous clinical trials, standardization of herbal products, and enhanced pharmacovigilance frameworks are essential to establish safe, effective, and patient-centered integrative healthcare strategies.
